Why the Inhaled Route Matters
In pulmonary fibrosis, lung tissue gradually stiffens and thickens, and the surface available for gas exchange shrinks away. Intravenous delivery has to survive systemic dilution before it ever reaches the lung, so only a small fraction arrives at the alveoli.
Nebulization works differently. Stem cell-derived extracellular vesicles measure roughly 30–150 nm, small enough to ride an aerosol all the way to the alveolar surface and build a local concentration where it is needed, while limiting exposure elsewhere in the body.
How the Study Was Designed
The study was carried out and published independently by a Tsinghua University research team. It used a randomized, double-blind, placebo-controlled design with 24 participants. Forced vital capacity (FVC) was the primary endpoint; six-minute walk distance (6MWD) and safety monitoring were secondary. The findings were published in a Nature-family journal. What the Numbers Showed
Against placebo, the treated group gained an average of 12.5% in FVC and 45 metres in six-minute walk distance, with a clinical response rate of 83.3%. No serious adverse events were recorded throughout the study.
